Submit a Claim - Claim Instructions

If you are a member of the Settlement Class and wish to receive an award under the Settlement, you must submit a valid Claim Form so that it is postmarked (if mailed) or submitted online no later than October 28, 2024.


How to File Online:

Unique ID and PIN required for login/submission. Please see the Claim Form sent to you for your Unique ID and PIN. If you do not have your Unique ID and PIN, please contact the Settlement Administrator at 1-888-209-4680 (toll-free).

How to File by Mail:

Fill out, sign, and date the Claim Form, then mail the Claim Form to the following address:

Remember, all Claim Forms must be submitted online or by mail. If by mail the Claim Form must be postmarked no later than October 28, 2024.
